Bring the completed forms, along with the necessary information as described below, to the Rhode Island Lottery, 1425 Pontiac Avenue, Cranston, RI 02920.

1) Rhode Island Lottery Sales License Application [135 k]

2) Gen. Disclaimer Form [11 k] (This must be completed along with the application)

3) The following will be needed with the Rhode Island Lottery Sales License application:

• 2 copies of applicant’s driver’s license
• $50.00 Non-Refundable Application Fee
• Copy of Permit to Make Sales at Retail License
• Voided check. (If opening checking rather than savings account.)
• Front page, back page and the “Disclaimer” must be completed and notarized.
• List two business references (vendors) where you buy your supplies, products, etc.
